What is the Small Employer Program and why is Vault different?

  • When you join the Small Employer program, you become an owner of the Vault Series C Captive—a collective of independent business owners sharing healthcare costs to reduce expenses. Instead of just paying premiums, you're taking ownership. And when claims are lower than expected, you can benefit through future savings and lower rates.

Want to Know if You're Eligible?

You must be an independent, self-employed business owner with NO employees.


You must have a Federal Tax ID Number.
 

Eligibility for the program is also determined using a health questionnaire. If your answers to any of the questions are “yes,” you might not be eligible for the Small Employer Program right now - but we’ll review your responses carefully and let you know your options.

Key Components of the Small Employer Program

Access to a Nationwide PPO Network

When you establish your self-insured health plan, you'll have access to the nationwide PHCS Practitioner & Ancillary Network, meaning you shouldn’t have to change your current provider. To search for in-network providers or check if your current provider is included, just click the button right above this section. If you choose an out-of-network provider, your claims will still be covered and priced at a fair and reasonable value through the program’s repricing vendor.

Prescription & Medication Coverage

The program uses FairosRx as its Pharmacy Benefits Manager. Fairos provides access to prescriptions, low-cost pharmacies, drug coverage, benefits information, manages mail-order prescriptions, and more.

Vault Cares Network

The Vault Cares Network connects you to elite providers for expert second opinions and advanced treatments—often beyond what traditional insurance covers. Each facility is hand-picked for clinical excellence, giving you access to world-class care, better outcomes, and total peace of mind when it matters most.

Vault Admin Services (Your in-house Third-Party Administrator)

VAS is our in-house Billing & Claims administrator that manages all aspects of your self-insured health plan. It provides customer support, education, manages your claims, eligibility, and sends out your health plan ID cards.

Mobile App

The Small Employer strategy program you access to Benefits Hero, your personal command center for everything health benefit related. This powerful, intuitive app puts your benefits at your fingertips, making it easy to find care, track spending, access ID cards, and more. It’s modern, effortless, and designed to simplify your healthcare experience like never before.

Telemedicine

Through our partnership with Clever Health, you get 24/7 access to smart, convenient virtual care—wherever you are. From primary care and mental health counseling to expert advice for your pets, licensed providers are just a tap away, making it easier than ever to take care of your health (and theirs) on your schedule.
